Case Management for Non-Medical Drivers of Health
Mona Mentors Inc. offers Case Management for children, adolescents, and adults with complex needs. Our goal is to connect clients with services like medical care, education, housing, employment, insurance, and more to help them achieve independence. Case managers coordinate treatment, advocate for clients, and monitor outcomes to ensure effective support.
Commercial health insurance and Medicare Case Management provide services to assist clients with serious health conditions in accessing appropriate care and support. These programs coordinate with clients, their families, caregivers, and medical providers to address health needs.
