What Is a DOT SAP Evaluation?
A DOT Substance Abuse Professional (SAP) evaluation is a federally mandated assessment required when a safety-sensitive employee violates DOT drug and alcohol regulations — including a positive drug test, a refusal to test, or an alcohol test result of 0.04 or higher. Watch our video to learn more.

The SAP Return-to-Duty Process
Initial SAP Evaluation
A thorough face-to-face or Video clinical assessment to evaluate the nature and extent of the substance use issue and determine appropriate follow-up education or treatment.
Education or Treatment
Completion of the SAP-recommended program — which may include substance abuse education, outpatient counseling, or a formal treatment program.
Follow-Up SAP Evaluation
After completing the recommended program, a second evaluation confirms compliance and determines whether the employee is ready to return to safety-sensitive duties.
Return-to-Duty Testing
The employer arranges a directly observed return-to-duty drug and/or alcohol test. A negative result allows the employee to resume safety-sensitive functions.
Follow-Up Testing Plan
The SAP prescribes an unannounced follow-up testing schedule (minimum 6 tests in the first 12 months, maxium 5 years) to monitor continued compliance.

How long does the SAP evaluation take?
The initial evaluation typically takes 60–90 minutes. A follow-up evaluation is shorter, usually 45–60 minutes, once the recommended program is completed.
Can the SAP evaluation be done via telehealth?
