SAP Evaluations Serving Arizona, Utah, Virginia & Maine
Mona Therapy is a federally qualified Substance Abuse Professional (SAP) provider serving DOT-regulated employees across Arizona, Utah, Virginia, and Maine. Whether you are a commercial truck driver in Phoenix, a pipeline worker in Salt Lake City, a maritime crew member in Portland, or a transit employee in Richmond, we provide the thorough, compliant SAP evaluation you need to begin the return-to-duty process.
A DOT SAP evaluation is federally required when a safety-sensitive employee violates DOT drug and alcohol regulations — including a positive drug test, a refusal to test, or an alcohol test result of 0.04 or higher. We offer flexible scheduling, including telehealth options where permitted by DOT guidelines, to minimize disruption to your work and life.
Serving Four States
Arizona
We serve DOT-regulated employees throughout Arizona, including Phoenix, Tucson, Mesa, Scottsdale, and Flagstaff. Arizona's extensive highway network — I-10, I-17, and I-40 — supports a large FMCSA-regulated trucking workforce, and the state's energy and pipeline sectors employ many PHMSA-regulated workers who may need SAP evaluations.
- Phoenix metro commercial truck and bus drivers
- Tucson and Flagstaff transportation workers
- Arizona pipeline and hazardous materials employees
- Transit and rail employees across the state
Utah
We serve safety-sensitive DOT employees across Utah, including Salt Lake City, Provo, Ogden, and St. George. Utah's booming logistics and distribution industry, combined with its significant mining and energy sectors, creates a large population of DOT-regulated workers who may require SAP evaluations.
- Salt Lake City and Provo commercial drivers and logistics workers
- Utah mining and energy sector PHMSA-regulated employees
- FTA transit employees in the Wasatch Front corridor
- Rural workers served via telehealth where permitted
Virginia
We serve safety-sensitive DOT employees throughout Virginia, including the Hampton Roads region, Northern Virginia, Richmond, Roanoke, and the Shenandoah Valley. Virginia's major transportation corridors — I-95, I-81, and I-64 — support a large FMCSA-regulated trucking workforce, and we are here to help those drivers navigate the return-to-duty process efficiently.
- Hampton Roads & Norfolk maritime and port workers
- Northern Virginia transit and rail employees
- Richmond and Roanoke commercial truck drivers
- Pipeline and hazardous materials workers statewide
Maine
We serve DOT-regulated employees across Maine, including Portland, Bangor, Augusta, and rural communities. Maine's fishing, maritime, and transportation industries employ a significant number of safety-sensitive workers who may require SAP evaluations, and we provide accessible, professional services to meet that need.
- Portland and Bangor commercial drivers and logistics workers
- Maine maritime and Coast Guard-regulated crew members
- Rural and remote workers served via telehealth where permitted
- Transit and railroad employees statewide
The SAP Return-to-Duty Process
Initial SAP Evaluation
A comprehensive face-to-face or telehealth clinical assessment to evaluate the nature and extent of the substance use issue and determine the appropriate education or treatment recommendation.
Education or Treatment
Completion of the SAP-recommended program — which may include substance abuse education, outpatient counseling, or a formal treatment program.
Follow-Up SAP Evaluation
After completing the recommended program, a second evaluation confirms compliance and determines whether you are ready to return to safety-sensitive duties.
Return-to-Duty Testing
Your employer arranges a directly observed return-to-duty drug and/or alcohol test. A negative result allows you to resume safety-sensitive functions.
Follow-Up Testing Plan
The SAP prescribes an unannounced follow-up testing schedule (minimum 6 tests in the first 12 months) to monitor continued compliance.
