Program Requirements and Structure
Duration
Programs typically last 8 to 12 weeks, or longer when required by court, probation, employer, or treatment recommendations.
Time commitment
Participants generally attend weekly sessions, with some programs requiring one or two sessions per week.
Aftercare
Some participants may be required to complete aftercare or follow-up services after the main program phase is completed.
Compliance and reporting
Attendance and progress reports may be sent directly to the supervising probation officer, court, employer, or referring agency when authorized or required.
Important Enrollment Steps
Before enrolling, verify your exact court order, probation requirements, employer mandate, or referral instructions with the appropriate supervising officer, court representative, employer, or agency. Mona Mentors PLLC offers virtual and in-person services when clinically appropriate and permitted by the applicable program requirements.
Payment for Therapy and Psychological Services
Payment is due at the time of service unless another written payment arrangement has been approved. Mona Mentors PLLC accepts major credit and debit cards. HSA and FSA funds may also be used for eligible psychotherapy and psychological assessment services. Payment methods are kept securely on file through the client portal and charged according to the payment policy.
Insurance, Deductibles, and Program Fees
Insurance benefits are quoted as a courtesy and are not a guarantee of payment. Clients are responsible for confirming their coverage, including whether Mona Mentors PLLC, Mona Ellis, or Mona Therapy is considered in-network or out-of-network under their specific plan.
Clients are responsible for all amounts not paid by insurance, including deductibles, copayments, coinsurance, non-covered services, denied claims, and any balance remaining after insurance processing. Deductibles and copayments are due according to the payment policy and may be collected at the time of service or after benefits are processed.
Program fees may not be covered by insurance. These may include Anger Management, Substance Abuse Outpatient, Parenting Skills Training, DOT or Non-DOT Substance Abuse Professional assessments, court-related evaluations, employer-mandated services, reports, letters, and administrative documentation. Unless Mona Mentors PLLC confirms in writing that a service will be billed to insurance, program fees are private-pay and must be paid directly by the client or responsible party.
Failure to provide accurate insurance information, update insurance changes, or receive payment from an insurance company does not waive the client's responsibility for payment. Mona Mentors PLLC may require payment arrangements before services continue, reports are released, or completion documentation is provided.
Good Faith Estimates
Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use insurance to pay for care have the right to receive a Good Faith Estimate of the expected cost of scheduled health care services.
A Good Faith Estimate explains the expected charges associated with your care. You may request one before scheduling or at any time during treatment.
For additional information about your rights under the No Surprises Act, visit CMS.gov. You may also contact Mona Mentors PLLC at 602-844-9803 with questions about the anticipated cost of services.
Cancellation and Missed Appointments
We understand that schedules change and that illness or unexpected circumstances may occur. Mona Mentors PLLC requires at least 24 hours' advance notice to cancel or reschedule an appointment. Late cancellations and missed appointments may be charged according to the cancellation policy reviewed when treatment begins.
Because missed appointments are not clinical services, cancellation and missed-appointment fees are generally not eligible for insurance reimbursement.
If you need to change an appointment, please contact your therapist as soon as possible.
