Need A DOT QUALIFIED SAP TO GET YOU BACK TO WORK?

The DOT SAP process is required for safety-sensitive employees who violate drug or alcohol rules. The steps are:

Step 1: After a violation, you’re removed from duty and your employer reports it to the FMCSA Clearinghouse.

Step 2: Schedule an evaluation with a DOT-qualified SAP (Mona Ellis, SAP Services). Evaluations are available in person or via telehealth for $490.00. Complete the client portal forms prior to your appointment date and time. 

Step 3: Complete the SAP recommended education or treatment and provide proof of services via client portal upload or confidential fax.

Step 4: Attend a follow-up evaluation for compliance and receive a Return-to-Duty (RTD) report.

Step 5: Pass a Return-to-Duty test and comply with at least six unannounced tests in the first year.

Required Documents:

• Valid CDL

• Employer contact details

• Violation details (test results/refusal)

• FMCSA Clearinghouse SAP request

These documents ensure verification of violation status and accurate reporting to federal databases.
